ABC Disability care has a team of Support Coordinators who can help you utilise your Plan Managed and Self Managed NDIS funding and connect you to service providers.
A Support Coordinator will support you to understand and implement the funded supports in your plan and link you to community and mainstream services. Support Coordinators will assist you to ‘optimise’ your plan ensuring that you are getting the most out of your funded supports.
A Support Coordinator may:
Connect you to service providers
Assist you to utilise your NDIS funding
Help you to plan and prepare for your NDIS review
Focus on supporting you to build your NDIS skills
Assist you to access the myplace portal
Support you to understand service agreements
Support you to have choice and control over your NDIS plan
